Opt-In and Consent

When you provide us with your mobile phone number and explicitly opt-in to our SMS program, you are consenting to receive text messages from City Mind NP.

  • How to Opt-In: You may opt-in through several methods, including:
    • Checking a box on our patient intake or consent forms.
    • Texting a specific keyword (e.g., “APPOINTMENT” or “YES”) to our designated short code or phone number.
    • Requesting SMS communications verbally during a conversation with our staff.
  • Message Frequency: The number of messages you receive will vary based on your interaction with us and the services you receive. For appointment-related messages, frequency depends on your scheduled visits. For promotional alerts, we aim to limit messages to a reasonable frequency (e.g., 2-4 messages per month).
  • Cost: Standard message and data rates may apply to any text messages you send or receive. You are responsible for all charges imposed by your mobile service provider. City Mind NP does not charge for the service itself.

Opt-Out and Stopping Messages

You can stop receiving SMS messages from us at any time.

  • To Opt-Out: Text the word STOP to the short code or phone number used to send you messages. You will receive a final confirmation message acknowledging that you have been unsubscribed and will no longer receive further text messages from that program.
  • Help Command: For help regarding our SMS service, text the word HELP to the short code or phone number.

Privacy and Confidentiality

City Mind NP is committed to protecting your privacy and the confidentiality of your Protected Health Information (PHI), as we are a healthcare provider governed by HIPAA.

  • Purpose of PHI: Text messages may contain PHI, such as appointment dates, times, and medication refill reminders. By opting in, you acknowledge that text messages are generally not encrypted and accept the risk of communicating this information via SMS.
  • Data Usage: We use the mobile phone number you provide solely for the purpose of communicating with you as described in this policy and our main Privacy Policy. We do not share your mobile number with unauthorized third parties for marketing purposes.
  • Security: We utilize secure, HIPAA-compliant platforms for transmitting PHI via SMS, but the security of the transmission ultimately relies on your mobile carrier and device security settings.

Liability and Disclaimer

City Mind NP is not responsible for delayed or undelivered messages. Delivery of text messages is subject to effective transmission from your mobile service provider. We rely on your mobile carrier for message delivery and cannot guarantee delivery at all times.

For urgent matters or emergencies, do not rely on SMS communication. If you are experiencing a mental health crisis, please call 911 or the National Suicide Prevention Lifeline at 988.
